Thrive Well Physical Therapy, Inc. accepts HSA, FSA, and cash/credit but does not accept or bill insurances directly. ThriveWell Physical Therapy, Inc. is considered an Out of Network (OON) Provider. If your insurance provides some reimbursement for OON, it will be the patient's responsibility to learn what documents are needed (such as pre-authorization from a doctor or superbill.) Upon request, I can provide superbills for insurance. I have made the decision to opt out insurances due to declining reimbursement rates and very high patient deductibles which meant patients were still paying a high price for less time and less care from their providers. Please note payment is due at the time of booking, and my cancellation policy is 24 hours.
